This form must be completed by all prom-goers, including invited guests of Wayland High School Juniors. Juniors may complete the form and attend Late-Night without attending prom. Students may not attend Late Night unless they are registered!
This registration form must be completed with a parent/guardian to sign acknowledgements and waivers.
